Registration for Hospital Data Review Site

Hospital access credentials are assigned to one contact person within your facility. After submitting this information and selecting your facility, you will receive your userid and password via e-mail. You may then return to this site to review the data on your facility collected by Subimo. Within this site, you will be able to comment on the information, and, in some cases, change the data. You can also complete the Subimo Hospital Survey to provide additional information to consumers in your market. The information you submit will not be integrated into Subimo's databases until your affiliation with your facility and responsibility for providing this information have been authenticated, and until Subimo receives a hardcopy version of the submission.

You facility's Medicare ID number is required to received access to this information. Also, please note that email addresses for 'aol', 'yahoo', 'msn' or 'hotmail' will not be accepted.
